Hoa Homeowner Forms
- ACC Improvement Request Form -- Helotes Park Terrace HOA members are required to submit an Architectural Improvement Request when making changes to their property. The HOA Board can require fees of up to $200 for specific improvements upon approval. See the form below for submitting requests to the HOA.
- Homeowner Info Sheet -- New homeowners can use this form to provide pertinent contact information required by the HOA
- Estate Sale Agreement -- A homeowner can submit a request to have an estate sale (not a garage sale) to be approved by the HOA Board
- Payment Plan -- A homeowner can submit a request for a payment plan in the event they are unable to pay their annual dues at the required time.
